การส่งเอกสารเพื่อตรวจสอบคุณสมบัติผู้สมัครสอบ ปี 2568 จะใช้ช่องทางอิเล็กทรอนิกส์ โดยแต่ละสถาบันรวบรวมเอกสารของผู้สมัคร ส่งเป็น Google drive link

การตั้งชื่อไฟล์เอกสาร:

<หมายเลข>-<ชื่อเอกสาร>_ชื่อผู้สมัคร.<นามสกุลไฟล์>

ตัวอย่าง:

  • 1-ใบสมัคร_ดาริกา.pdf = หลักฐานสมัครจากระบบแพทยสภา
  • 2-หนังสือรับรอง_ดาริกา.pdf = หนังสือรับรองจากสถาบัน
  • 3-01ACLS_ดาริกา.jpg = certificate ACLS
  • 4-01MEM_ดาริกา.jpg = หลักฐานโอนเงินค่าสมัครสอบ

เอกสารประกอบการสมัครสอบ แบ่งออกเป็น 4 หมวด:

1. เอกสารจากระบบแพทยสภา 1 ชุด

  1. 1-ใบสมัคร_Name = หลักฐานการสมัครสอบผ่านระบบแพทยสภา

2. หนังสือรับรองจากสถาบันที่ฝึกอบรม 1 ชุด

  1. 2-หนังสือรับรอง_Name = หนังสือรับรอง กำลังจะสำเร็จการฝึกอบรมแพทย์ประจำบ้าน หรือปฏิบัติงานประจำสาขาในปี 2568 โดยระบุรายละเอียด:
    1. ระยะเวลาผ่านการฝึกอบรมความตามหลักสูตร (ไม่น้อยกว่า 144 สัปดาห์)
    2. ระยะเวลาผ่านการฝึกอบรมเวชปฏิบัติฉุกเฉิน (ไม่น้อยกว่า 80 สัปดาห์)
    3. ระยะเวลาฝึกอบรมด้านอำนวยการและสั่งการปฏิบัติการฉุกเฉินการแพทย์ (ไม่น้อยกว่า 8 สัปดาห์)
    4. ผ่านการประเมินกิจกรรมวิชาชีพที่เชื่อถือไว้ใจได้ (EPA)
    5. ทำหัตถการครบตามกำหนด (ตามเกณฑ์ 2561)
    6. สถาบันเห็นสมควรให้เข้าสอบ

3. หลักฐานผ่านการฝึกอบรม

  1. 3-01ACLS_Name = ใบรับรองผ่านการฝึกอบรมการกู้ชีพชั้นสูง (ACLS)
  2. 3-02PALS_Name = ใบรับรองผ่านการฝึกอบรมหลักสูตรการช่วยชีวิตขั้นสูงในเด็ก (PALS)
  3. 3-03ATLS_Name = ใบรับรองผ่านการฝึกอบรมการช่วยชีวิตขั้นสูงสำหรับผู้บาดเจ็บ (ATLS)
  4. 3-04EPA_Name = ใบรับรองการผ่านการประเมินสมรรถนะ EPA
  5. 3-05หัตถการ_Name = ใบรับรองการทำหัตถการครบตามกำหนด (ตามเกณฑ์ 2561) ใน App tcep

การฝึกอบรม และการประเมิน ที่ใช้ระบบ e-learning จะตรวจสอบจากส่วนกลาง ได้แก่

  • Resuscitative procedure
  • Emergency medical toxicology
  • Emergency medical director
  • Comprehensive Life Support exam
  • การประเมินผลงานวิจัย

โดยผู้สมัครจะต้อง กดรับใบรับรองการผ่าน (certificate) ในระบบ e-learning เมื่อรับใบรับรองแล้วไม่ต้องส่งเอกสารเพิ่มเติม
